Findings: According to the documentation, the facility failed to follow doctor’s orders to provide a cushion for the Alleged Victim’s (AV) wheelchair to prevent skin breakdown. On or about October 30, 2024, the AV received orders for a cushion. There was no documented evidence the facility ordered the cushion until approximately December 16, 2024. The failure to follow doctor’s orders and provide a seat cushion put the AV at risk of harm to develop or have their current wounds worsen, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
