Findings: Witness #1 IW1) is known to engage in altercations, and interventions in place were to re-direct AV. On or about December 14, 2024, the Alleged Victim (AV) and W1 engaged in an altercation, hitting and shoving each other. AV suffered a skin tear from the altercation. The facility was short staffed at the time of the incident with only one staff available. The facility's failure to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
