Findings: The Alleged Victim (AV) and Witness 1 (W1) both rely on the facility for their care and have a history of resident to resident physical altercations. W1 is known to be possessive of his/her room and personal items/space. According to an investigation, on or about October 25, 2024, AV walked near W1's room, resulting in W1 pushing AV out of the way, placing AV at risk for harm. The facility failed to implement interventions and/or appropriately care plan for AV's and W1's known behaviors and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
