Findings: On or about December 18, 2024, The Alleged Victim was sitting in his/her wheelchair when witness #1 (W1) grabbed and shoved AV by the shoulder, causing pain to AV. W1 is known to have behaviors and AV is to be supervised by staff at all times, to ensure AV's safety. On this day, no staff were assigned to be responsible for AV. Repsondent failed to provide a safe environment for AV. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
