Findings: Alleged Victim (AV) has a resident trust account that is kept at the front desk in an envelope in a locked drawer. AV and AV’s personal caregiver withdraw funds from the account when they go out on Thursdays. On or about December 12, 2024, the money in the envelope was counted by facility staff and there was $162.04 in the envelope. On or about December 19, 2024, AV’s personal caregiver asked for money to take AV out for the day when staff found $120 was missing, leaving AV with $42.04. The drawer was found unlocked sometime between the dates of December 12, 2024, and December 19, 2024, coinciding with the timeframe AV’s money was found to be missing. AP1 has reimbursed AV for the amount missing from AV’s account. An Unknown Alleged Perpetrator 2 (AP2) and the facility (AP1) are responsible for not securing AV's funds resulting in theft, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
