Findings: The Alleged Victim (AV) relies on the facility for his/her care and has a history of falling, especially when AV is not feeling well. On or about December 18, 2024, fell out of his/her recliner and had to call staff for help. According to an investigation, despite the facility being aware that AV was at an increased risk of falls when not feeling, no additional interventions were put in place to ensure AV's safety. The facility failed to provide services, to include implement effective interventions to mitigate AV’s falls and monitoring AV for safety, resulting in a risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
