Findings: On or about December 22, 2024, Alleged Perpetrator #2 (AP2) was assisting the Alleged Victim (AV) with cares when AV became agitated and began yelling at AP2. AP2 pushed AV and poked him/her in the chest, telling AV "you don't do that". Another staff intervened and took over cares of AV. Approximately 15 minutes later, AV was still agitated and followed AP2 into another residents room. AP2 pushed AV out of the room and locked the door behind AV. After AP2 pushed AV out of the room, it was discovered that AV suffered a skin tear due to the interaction. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility's failure to protect AV from physical abuse is a violation of Oregon Administrative Rules.
