Findings: The Alleged Victim (AV) resides at the facility and has a recent history of a previous fall. According to an investigation, on or about December 23, 2024, AV fell out of bed, resulting in treatment at the emergency room, pain and a fractured clavicle. AV had a previous fall out of bed on December 20, 2024. AV's care plan had not been updated following the fall on December 20, 2024, to reflect AV's need for fall interventions. The facility failed to properly care plan, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
