The facility failed to provide appropriate services according to the AV's needs, relating to their number of falls with lack of appropriate interventions. According to an investigation, the AV experienced multiple unwitnessed falls between approximately November 5, 2024, and December 18, 2024, resulting in pain and injuries. The failure is a violation of resident rights, is considered neglect of care, and constitutes abuse.
