Findings: On or about December 27, 2024, Alleged Perpetrator 2 (AP2) was observed by facility staff calling Alleged Victim (AV) a "fat pig" while AP2 was assisting AV with his/her Activities of Daily Living. At time of incident, AP2 was working in the role of AV's caregiver assisting AV with his/her nightly care needs. AP2 was observed to be agitated and frustrated, as AP2 yelled at AV, saying "if {you} weren't such a fat pig it would be so much easier to change {you}". AP2's actions caused AV to be visibly upset and emotional. AP2 verbally/emotionally abused AV by calling AV derogatory names causing AV emotional harm. The facility failed to protect AV from verbal abuse which is considered a violation of Oregon Administrative Rules.
