Findings: The Alleged Victim (AV) relies on the facility for his/her care and medication management. According to an investigation, on or about December 16, 2024, AV began experiencing a change of condition, including confusion, not feeling well, and having difficulty with expression and thoughts. AV was not sent to the hospital until approximately December 26, 2024. At the hospital, AV was diagnosed with acute kidney injury due to medication induced hypotension and dehydration, which was identified by hospital staff to be near catastrophic. The facility failed to appropriately assure timely medical treatment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
