- Findings
- Witness 1 (W1) goes out into the community with W1's support person and has a history of being agitated when returning. W1 has a history of resident-to-resident altercations with other residents and with Alleged Victim (AV). W1 and AV are roommates. W1 service plan dated on or around December 26, 2024, indicates staff to notify med tech and nurse if behaviors or agitation are apparent and interventions aren't working. W1 interventions, medications scheduled and PRN order for agitation; Make sure W1 is provided with W1's own space in W1's room in a low stimulation environment; If staff observe W1 showing signs of escalating behaviors of anxiety, agitation, or aggression towards AV, they are to promptly remove AV from the area for AV's safety. On or about December 29, 2024, W1 was sitting in the dining area getting ready for dinner. W1 went into the living room and sat next to AV. W1 became agitated and started slapping towards AV, resulting in AV and W1 slapping each other. W1 was already upset before the incident with AV, as W1 had been out that day with W1's support person. The facility failed to recognize those sings and implement interventions. The facility failed to follow W1 care plan If staff observe W1 showing signs of escalating behaviors of anxiety, agitation, or aggression towards AV, they are to promptly remove AV from the area for AV's safety, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
