Findings: The Alleged Victim (AV) is a known elopement risk and is care planned to for 2 hour safety checks. If staff do not see AV at dinner or AV misses a med pass, staff are to contact the administrator or Nurse. On or about December 29, 2024, AV was not present for dinner, missed his/her med pass, and was not signed out of the facility. At about 9:21 pm, a supervisor was contacted regarding AV being missing. Per video footage, AV left the facility at 3:15 pm and was found at 9:00 am the next day. AV experienced back pain from sleeping outside on the ground. The facility's failure to follow AV's care plan placed AV in unreasonable discomfort and at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
