Findings: The Alleged Victim (AV) was a known fall risk. On or about December 14, 2024, AV suffered two separate falls. AV suffered another fall on December 15, 2024, resulting in pain in AV's hip. AV was transported to the hospital, but there was no found cause of AV's hip pain. The facility's failure to properly care plan to ensure AV's safety from falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
