Findings: The facility failed to provide a safe environment for the Alleged Victim (AV) who has a known history of elopements and exit seeking behavior. On or about December 30, 2024, the facility held a fire drill. During the fire drill, another resident’s family member was leaving the facility and when they exited the facility, the AV followed them out the door. The facility staff did not notice the AV leaving the secured memory care facility. The AV was found outside of the secured memory care by staff from another unit and was escorted back to the memory care unit. The facility failed to provide a safe environment allowing the AV to leave the secured memory care without supervision, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
