Findings: The Alleged Victim (AV) and Witness #1 (W1) were known to engage in altercations and were to be kept separate in the dining area. On or about January 3, 2025, AV confronted W1 for taking multiple drinks from the refrigerator. AV hit W1 with a TV Tray. W1 hit AV in the face causing a split lip. The facility's failure to ensure enough staff were present to ensure the safety of residents is a violation of resident rights, is considered neglect of care and constitutes abuse.
