Findings: The Alleged Victim (AV) relies on the facility to provide care, services and a safe environment. According to an investigation, on or about November 11, 2024, AV suffered an unwitnessed fall in their room. AV complained of pain that next day, and AV was unable to find any staff to assist AV. AV went downstairs and found a caregiver but was re-directed back to their floor in the facility. AV called a family member, and they called the facility to get AV assistance. AV was sent out to the hospital and found to have a fractured hip. The facility failed to provide care and services to AV timely, which is a violation of resident rights and is neglect which constitutes abuse.
