Findings: On or about January 7, 2025, Alleged Perpetrator #2 (AP2) was trying to wake up the Alleged Victim (AV) to assist with taking him/her to the restroom. AV is known to be difficult to wake up. AV was seen by staff using his/her pointer finger and middle finger to repeatedly thump AV on the head to wake AV up. AV was observed by staff coming out of the restroom by staff holding the side of their head and a red mark was noticed on AV's head. Later that same evening, AP2 was seen pulling on AV's arms, cussing and raising his/her voice at AV to wake AV up. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical and verbal abuse. The facility's failure to protect AV from physical and verbal abuse is a violation of Oregon Administrative Rules.
