Findings: Alleged Victim (AV) is independent in ambulation without assistive devices. AV did not have a pattern of falls and AV was not a fall risk. On January 8, 2025, AV was entering the lobby area and AV tripped over a vacuum power cord which resulted in AV falling face first on to the floor. As a result, AV ended up with a swollen left eye and a nosebleed and was sent to the hospital for evaluation. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
