Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim (AV) after they returned to the facility for a fall which resulted in a fracture. On or about May 05, 2023, the AV returned to the facility after an approximately four months stay at a skilled nursing facility after falling and sustaining a hip fracture. On or about May 06, 2023, the AV experienced a fall in their bathroom. The facility failed to update the care plan when the AV returned from the skilled nursing facility to reflect new care needs, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
