Findings: The Alleged Victim (AV) and Witness #1 (W1) are known to engage in altercations with each other. AV had an intervention in place to re-direct him/her from W1, as W1's presence appeared to be a trigger for AV. On or about January 9, 2025, AV was sitting at the dining room table with W1. AV threw a pack of cookies at W1 and W1 threw the cookies back at AV, hitting him/her in the head. The facility's failure to follow interventions lead to AV and W1 engaging in an altercation. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
