Findings: The Alleged Victim (AV) relies on the facility for care and services. According to an investigation, the AV had suffered multiple falls with injury that the facility had not previously reported. The care plan did not have interventions put in place to mitigate AV's risk of injury with falls. The facility failed to implement interventions to AV's care plan to help keep AV safe from continued falls, which is a violation of resident rights, neglect of care which constitutes abuse.
