Findings: The Alleged Victim (AV) relies on the facility for his or her safety. On or about January 17, 2025, AV followed a contracted staff member out of the secured facility into the lobby. AV did not want to go back into the secure unit and was upset, swinging fists, trying to kick staff, and AV was agitated while staff were trying to redirect AV back into the secured area of the memory care unit. The facility failed to provide a safe environment, by ensuring everyone was trained on how to exit the secured unit, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00279 $375.00 fine assessed
