Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV was noted to require assistance with incontinence and noted to be at risk for falls related to impaired balance. According to an investigation, AV experienced approximately three unwitnessed falls between December 15, 2024, and January 3, 2025. On or about January 18, 2025, the AV was found in their bathroom, bent over their walker with their head stuck upside down in a shower chair. The AV’s face was covered in mucus and there was dried urine and feces on the floor. The AV was not able to respond to staff and was sent to the hospital. The facility failed to provide appropriate services according to AV’s needs, relating to care plans and lack of appropriate interventions and instructions, to mitigate the risk of injury due to AV's increase in falls, which is a violation of resident rights, is neglect of care, and constitutes abuse.
