Findings: The Alleged Victim (AV) is a known fall risk with a history of falls. The AV was placed on hospice services, on or about January 7, 2025, due to a decline in condition. According to an investigation the AV experienced multiple falls between approximately December 7, 2024, and January 16, 2025. The AV had another fall on or about January 19, 2023, resulting in a skin tear to the left elbow. The facility failed to provide interventions to mitigate the risk of falls due to the AV's decline in condition and increase in falls, which is a violation of resident rights, is neglect of care, and constitutes abuse.
