Findings: On or about January 23, 2025, the Alleged Victim (AV) was riding in the facility bus, driven by Alleged Perpetrator #2 (AP2). On this day, AP2 was driving, AV was in the back of the bus in his/her wheelchair when AP2 started climbing a hill with the bus. AV fell backwards and hit his/her head on the van, causing a hematoma to AV's head. AP2 failed to secure AV's wheelchair in the van, causing AV to tip backwards. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure to ensure documented training for AP2 driving the facility van is a violation of resident rights, is considered neglect of care and constitutes abuse.
