Findings: The Alleged Victim (AV) relies on the facility to provide assistance with care and is care planned for stand by assistance with showers. According to an investigation, on or about January 25, 2025, the Alleged Perpetrator 2 (AP2) left the AV alone in the shower assistance and without informing other staff of the AV's need for assistance. AP2's actions caused the AV to experience fear and put the AV at risk harm. AP2’s actions are a violation of resident rights, considered neglect, and constitutes abuse. The facility failed to ensure the AV was not neglected which violates Oregon Administrative Rules.
