Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, on or about January 25, 2025, AV had a fall after coming out of AV’s room while shaking the railing in the hallway and stepping backwards. AV was crying out in pain and holding onto AV's arm. Staff took AV's vitals, performed range of motion and assisted AV into a wheelchair. AV was taken by a family member to the hospital and had sustained a fractured shoulder. Emergency services were not contacted following the fall. The facility failed to follow provide appropriate care, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
