Findings: The Alleged Victim (AV) relies on the facility to provide appropriate care and services. The AV has a history of falls, a history of not using their call light, and has a health issue that causes AV to become dizzy at times. According to an investigation, between February 2, 2025, and February 4, 2025, AV suffered three falls, resulting in multiple injuries, including back pain and a head injury. The facility failed to ensure the care plan was followed and/or implement interventions to mitigate AV’s increasing fall risk, resulting in repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
