Findings: Alleged Victim (AV) lives at Respondent’s facility and relies on facility staff to ensure AV's basic care, safety, and supervision requirements are met. AV's service plan reflects that AV has poor recent memory, requires staff assistance with transfers due to weakness. AV has a history of attempting to transfer without staff assistance. On or about September 10, 2024, AV self-transferred from AV's bed without staff assistance, resulting in a skin tear to AV's left elbow. The facility failed to develop interventions in AV's Service Plan to address AV's history of attempting to transfer without staff assistance. Respondent failed to develop and implement suitable interventions, and this constitutes abuse by neglect.
