Findings: Alleged Victim (AV) lives at Respondent’s facility and relies on facility staff to ensure AV's basic care, safety, and supervision requirements are met. AV has a history of attempting to transfer without staff assistance, including an incident that occurred on September 10, 2024. AV's service plan reflects that AV has poor recent memory, requires staff assistance with transfers due to weakness. On or about September 28, 2024, AV fell when attempting to self-transferring into AV's wheelchair, hitting AV's head, resulting in a bump on the back of AV's head. The facility failed to develop interventions in AV's Service Plan to address AV's history of attempting to transfer without staff assistance. Respondent failed to develop and implement suitable interventions, and this constitutes abuse by neglect.
