Findings: On or about January 23, 2025, Alleged Perpetrator #2 (AP2) was upset and swore while assisting the Alleged Victim (AV) with cares. While AP2 was not speaking to AV, AP2 did make statements including swearing in front of AV, causing AV unreasonable discomfort and experiencing verbal abuse. AP2's actions are a violation of resident rights, are considered neglect of care and constitutes abuse. The facility's failrue to protect AV from verbal abuse is a failure of Oregon Administrative Rules.
