Findings: The Alleged Victim (AV) relies on the facility to provide a safe environment. The AV is known to wander and has a history of exit seeking behaviors. According to an investigation, facility staff did not make sure the door locked when entering the facility and AV was able to elope from the facility through the unlocked door, posing a risk of harm. The facility failed to provide a safe environment for AV by not ensuring the door was locked, which is a violation of resident rights and is neglect of care, which constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) was investigated and determined to not be substantiated.
