According to an investigation, between December 2, 2024, and January 21, 2025, AV had five injuries of unknown origin and were related to unwitnessed falls. The facility failed to implement meaningful interventions to AV's care plan to prevent AV from falling. This is a violation of resident rights and is considered neglect which constitutes abuse.
