Findings: The Alleged Victim (AV) was care planned to have his/her food cut up into small pieces to ensure his/her safety from choking. On or about February 4, 2025, AV was given the wrong tray of food at meal time by a newer staff member by accident. Another staff member realized the mistake and went to check on AV and realized he/she was choking. Staff quickly dislodged the food from AV's throat. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
