Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls. On or about November 18, 2024, AV had a fall resulting in a skin tear. According to an investigation, the intervention listed after the October 24, 2024, fall is to ensure AV’s wheelchair is locked. This intervention had already been put in place after a prior fall on September 20, 2024. There were no interventions listed after the fall on October 28, 2024, or the two falls on October 30, 2024. The facility's failure to properly care plan for falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
