Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls. AV had a fall on or about October 28, 2024, and then two additional falls on October 30, 2024, putting AV at risk for harm. According to an investigation, the intervention listed after the October 24, 2024, fall is to ensure AV’s wheelchair is locked. This intervention had already been put in place after a prior fall on September 20, 2024. After the fall on October 28, 2024, and the first fall on October 30, 2024, there were no additional interventions put in place. The facility's failure to properly care plan for falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
