Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls. On or about October 7, 2024, AV had a fall resulting in a head injury with skin discoloration. According to an investigation, there were no new interventions documented after AVs fall on September 25, 2024. The facility's failure to properly care plan for falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
