Findings: The Alleged Victim (AV) relies on the facility for their care. According to an investigation, on or about January 26, 2025, AV suffered an unwitnessed fall. The AV did not express pain at the time of the fall, but on or about January 28, 2025 AV was expressing pain to the facility staff. The facility failed to have AV evaluated until February 4, 2025, and AV was taken to the hospital on February 5, 2025, by the AV’s family. It was determined that AV suffered an ankle fracture from the fall. The facility failed to provide appropriate and timely follow up care to AV after the fall, which is a violation of resident rights, is considered neglect of care, which constitutes abuse.
