Findings: According to the documentation, the facility failed to properly care plan for Witness 1’s (W1) behaviors of inappropriate touching. W1 has a history of touching other residents inappropriately on their private areas. On or about February 05, 2025, W1 was found standing over the Alleged Victim (AV) in AV’s apartment. W1 was redirected out of the AV’s room. After W1 left the room, the AV notified staff W1 was touching their chest and bottom. Staff are to keep an eye on W1 and redirect them when they enter another residents room. The facility failed to properly care plan appropriate interventions to prevent known inappropriate behaviors towards other residents, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
