Findings: The Alleged Victim (AV) relies on the facility for care and services. AV’s service plan states that AV requires one person assistance with bathing. According to an investigation, on or about January 21, 2025, it was reported that AV had a malodor to their body and when the concerns were brought to the facility it was discovered that AV had not had a shower in over three weeks. AV presented with dry, cracked skin, an odor, and unwashed hair, which resulted in a loss of dignity for the AV. The facility failed to provide basic care and services to AV, which is a violation of resident rights, neglect of care, and constitutes abuse.
