Findings: On or about September 12, 2023, the Alleged Victim (AV) suffered a fall and was transported to the hospital and diagnosed with a fractured femur. Upon return from the hospital, AV was to received physical therapy. Documentation was sent to AV's POA for approval. Physical Therapy was ordered, however, it was ordered for the wrong resident. AV never received physical therapy, leading to AV to not be able to ambulate in his/her wheelchair. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00217 $2500.00 fine assessed
