Findings: The Alleged Victim (AV) is a known fall risk. On or about November 26, 2024, AV slid out of bed and sustained a skin tear due to an object protruding from AV's bed. AV was to have on non slip socks, which he/she did not and a bed alarm in place, however, the bed alarm was not working. The facility's failure to ensure AV was safe from falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
