Findings: The Alleged Victim (AV) relies on the facility to ensure resident rights and safety. Witness 1 has a known history of sexually inappropriate behavior and touching of others, including the AV. Interventions were implemented to keep W1 away from opposite gender residents. W1 was prescribed medications for the inappropriate sexual behavior. W1 was not consistently administered their behavior medications due to unavailability and interventions were not implemented or effective. According to an investigation, on or about February 22, 2025, W1 was noted to touch the AV in a sexually inappropriate way, causing the AV significant stress and disruption of sleep. The facility failed to appropriately care plan and ensure a safe medication system, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
