Findings: The Alleged Victim (AV) and Witness #1 (W1) share a room and have had altercations in the past. AV does not like it when others touch his/her things. W1 likes to take items that don't belong to him/her. Staff are to observe W1 for signs of escalating and remove W1 from AV. On or about February 20, 2025, AV and W1 were advised to stay in their rooms. W1 was touching AV's things, AV treid to retrieve their items from W1 when W1 punched AV in the face, causing AV to fall to the floor and have a laceration to his/her lip. AV went to the hospital for observation for high blood pressure. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
