Findings: The Alleged Victim (AV) relies on the facility for their care, is known to wander into other residents' rooms and sleeping in unoccupied beds and was care planned for 1:1 care. According to an investigation, on or around, February 21, 2025, AV was found on the floor by staff, resulting in a humerus fracture to the left arm. Prior to the fall, AV was sleeping on an unoccupied bed in another resident's room and was not being monitored with 1:1 supervision. The facility failed to provide appropriate services, relating to care planning, which is a violation of resident rights, is neglect of care and constitutes abuse.
