Findings: The Alleged Victim (AV) relies on the facility for their care. According to an investigation, AV experienced exit seeking behaviors between March 1, 2025, and March 4, 2025. On or about March 5, 2025, at approximately 5:00am, it was discovered that AV was missing from the facility. AV was located approximately two hours later, roughly three miles away from the facility, placing AV at risk of harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
