Findings: The Alleged Victim (AV) relies on the facility for assistance with transfers, which includes use of a gait belt. According to an investigation, on or about February 28, 2025, AV had a hard time pivoting and standing without AV’s right foot sliding from AV’s case and staff struggled to change AV’s brief while lifting the gait belt, resulting in a skin injury. The facility failed to provide a safe environment and ensure that staff were properly trained in the use of a gait belt, which is a violation of resident rights, is neglect of care which constitutes abuse.
