Findings: The Alleged Victim (AV) relies on the facility to provide a therapeutic diet to prevent choking. According to an investigation, on or about December 29, 2024, the AV was served food that was not the appropriate texture. As a result, the AV choked on the food. The facility failed to provide the appropriate therapeutic diet to the AV, which is a violation of resident’s rights, is considered neglect of care, and constitutes abuse.
