Findings: The Alleged Victim (AV) relies on the facility for care and has a recent history of falls. According to an investigation, on or about September 8, 2024, AV had approximately two (2) separate falls, one in AV’s room and the other in the common living area, resulting in AV hitting their head and crying out in pain. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
